COMPOSITION:
Each gram cream contains:
Clobetasol propionate 0.05%

PHARMACOLOGY:
Clobetasol propionate is a local anti inflammatory and immunosuppressive agents which acts by inhibiting the adherence of neutrophils and monocyte-macrophages to the capillary endothelial cells of the inflammed areas.

INDICATIONS:
Clobetasol propionate is a topical corticosteroid used in short-term treatment of the more resistant dermatosis such as psoriasis, recalcitrant eczema, lichen, planus, discoid lupus erythematosus and other conditions which do not respond satisfactorily to less active steroids.

DOSAGE AND ADMINISTRATION:
Apply thinly to the affected skin areas once or twice daily.

CONTRAINDICATIONS:

• Cutaneous infections such as impetigo, tinea corporis and herpes simplex. 
• Acne vulgaris, rosacea
• Neonatus
• Patients hypersensitive to Clobetasol propionate or other corticosteroids.

ADVERSE EFFECTS:

• Excessive use over large surface areas may augment systemic absorption, producing adrenal suppression, Cushing's syndrome, hyperglycemia and hypertension.
• Burning sensation, itching, atrophic changes of the skin, irritation, dryness, folliculitis, hypertrichosis, acneform eruptions, hypopigmentation, perioral dermatitis, allergic contact dermatitis, skin maceration, secondary infections, striae and miliaria.

WARNINGS AND PRECAUTIONS:

• Administration to pregnant and lactating women is not recommended. Avoid prolonged continuous use especially in infants and children, as adrenal suppression may occur.
• Patients receiving large doses of corticosteroids applied to large surface areas for extended periods of time should be evaluated for evidence of HPA axis suppression. If HPA axis suppression occurs, reduce the frequency of application or substitute with a less potent corticosteroid.
• Children may absorb proportionally large amounts of topical corticosteroids and thus be more susceptible to topical corticosteroid-induced HPA axis suppression and Cushing's syndrome. Usage in children under 12 years of age is not recommended.
• If irritation develops, topical corticosteroids should be discontinued and appropriate therapy should be instituted.
• In the presence of dermatological infections, the use of an appropriate antifungal or antibacterial agent should be instituted.
• Observation of the patient is important if corticosteroids are used to treat certain areas of the body, such as the face, groin and axillae, as these areas are more prone to atrophic changes.
• As with other topical corticosteroids, the drug should not be used in the treatment of rosacea and perioral dermatitis. Topical corticosteroids in general should not be used in the treatment of acne. Nor should the drug be used as a single therapy in plaque psoriasis over large areas.

KOMPOSISI:
Tiap gram krim mengandung:
Klobetasol propionat 0,05%

CARA KERJA OBAT:
Klobetasol propionat merupakan anti-inflamasi lokal dan imunosupresan yang bekerja dengan menghambat penempelan netrofil dan monosit makrofag pada sel-sel endotel kapiler pada daerah yang meradang.

INDIKASI:
Klobetasol propionat merupakan kortikosteroid topikal untuk pengobatan jangka pendek terhadap dermatosis yang lebih resisten terhadap steroid yang kurang kuat, seperti psoriasis, ekzema rekalsitran, lichen planus, lupus eritematosus diskoid, dan keadaan lain dimana pemberian steroid yang kurang kuat tidak memuaskan.

ATURAN PAKAI:
Dioleskan tipis-tipis pada kulit yang sakit 1 - 2 kali sehari.

KONTRA INDIKASI :

• Infeksi kutan seperti impetigo, tinea korporis dan herpes simpleks. 
• Akne vulgaris, rosacea.
• Neonatus.
• Pasien yang hipersensitif terhadap Klobetasol propionat atau kortikosteroid lain.

EFEK SAMPING :

• Untuk pemakaian yang berlebihan pada bagian tubuh yang luas dapat terjadi absorbsi sistemik yang menyebabkan supresi adrenal, Cushing's syndrome, hiperglikemik dan hipertensi.
• Rasa terbakar, gatal, atropi kulit, iritasi, kekeringan, folikulitis, hipertrikosis, akneform erupsi, hipopigmentasi, dermatitis perioral, dermatitis kontak alergik, maserasi kulit, infeksi sekunder, striae dan miliaria.

PERINGATAN DAN PERHATIAN:

• Tidak dianjurkan pemberian pada wanita hamil dan menyusui.
• Penderita yang mendapat kortikosteroid dengan dosis besar, pemakaian pada permukaan yang luas dan dalam jangka lama sebaiknya dievaluasi terhadap penekanan HPA. Jika terjadi penekanan HPA, kurangi frekuensi pemakaian atau substitusi dengan kortikosteroid yang kurang kuat.
• Pada anak-anak secara proporsional mungkin mengabsorbsi kortikosteroid topikal lebih besar, sehingga kemungkinan terjadi penekanan HPA, Cushing's syndrome juga lebih besar.
• Tidak dianjurkan penggunaan pada anak-anak di bawah usia 12 tahun.
• Bila dalam pengobatan dengan kortikosteroid topikal terjadi iritasi, sebaiknya pengobatan dihentikan dan berikan terapi pengganti yang sesuai.
• Jika terjadi infeksi dermatologis, berikan obat-obat anti fungi atau anti bakteri yang sesuai sebagai terapi pengganti.
• Penting dilakukan pengawasan terhadap pasien terutama apabila kortikosteroid dipakai pada daerah-daerah tertentu, seperti : muka, pangkal paha, dan ketiak karena lebih mudah terjadi perubahan atropi dibanding bagian badan yang lain.
• Seperti halnya dengan kortikosteroid yang lain, obat ini tidak boleh digunakan pada pengobatan rosacea dan dermatitis perioral. Kortikosteroid topikal pada umumnya tidak digunakan untuk jerawat atau sebagai satu-satunya terapi pada psoriasis plak yang meluas.
